   • Health Examination Report  
   • Bill of Academic Fees
   • Student’s Pledge
   • UiPS Membership
   • Identification Card (I/C) (Certified true copy) 
   • Master / Bachelor Degree Certificate (Certified true copy)
   • Academic Transcripts (Certified true copy)

STEP 3
Payment of the bill can be made on or before the registration day.
